Celgene Patient Support® is a free and personal service that helps
your patients access the Celgene medication you have prescribed.
A single Celgene Patient Support® Specialist is assigned to your
office and can provide you and your patients with a full range of
financial assistance options and insurance-related services
CELGENE COMMERCIAL CO-PAY PROGRAM Reduced co-pay responsibility of $25 or less for eligible patients with commercial or private insurance (including healthcare exchanges)
CO-PAY ASSISTANCE THROUGH THIRD-PARTY ORGANIZATIONS For patients who have government-sponsored insurance, such as Medicare, your Specialist can help identify third-party organizations to help with co-pay costs
CELGENE FREE MEDICATION PROGRAM Free medication may be available for eligible patients who are uninsured or underinsured
Transportation Assistance Help with locating financial assistance for transportation costs through third-party foundations
• Foundations can help with the costs of gas, parking,
   tolls, etc
INSURANCE RELATED SERVICES Your Specialist is available to assist with each of the
following steps in obtaining insurance approval for
Celgene medications
• Conducting a benefits investigation
• Guiding your office through the prior
  authorization process
• Providing appeals support if you receive a denial
